5.	Probationer requests:
•	Kellie Roring, terminate therapy
•	Kay Dellinger, request for general supervision
•	Debbie Kilgrow, request to be allowed to work at Heartwood Home Health in the position of Admission RN.  
•	Michelle Peterson, requesting early termination of probation.
•	Lacy Tanner, therapist indicates she has met her treatment goals; and a request to be allowed to work at the 5 minute clinic 16 hours per week.
•	Michael Ross, request to amend the minimum number of hours worked per week.  Therapist indicates he has met his treatment goals.  
•	Katie Roach, therapist indicates she has reached maximum benefit from outpatient groups; request for access to controlled substances.  
•	Susan Cluff, amend Order for 12-step meetings only.
•	Diane Crossely, amend Order for 12-steps meetings only.
•	Sarah Cormier, request for early termination of probation.  
•	Kaylyn Thompson, request for general supervision; request for access to controlled substances; request to allow her to call in prescription orders to the pharmacy.  
•	Stephanie Thomas, request to have an OB/GYN added to list of prescribing practitioners. 
6.	NCSBN training
